BEFORE YOU PRESS THE SUBMIT BUTTON, please print two copies of this form. Include one copy with your payment and keep one copy for your records.
  • All net proceeds will be used to support worthy charitable activities.
  • This tournament is open to teams from all Divisions.
  • If your team does not play USSSA sanctioned softball, there will be a $20.00 sanction fee.
  • All tournament slots are filled on a first come, first served basis.
  • No entry is considered complete until the entry fee is received.
  • All special requests for tournament times will be given priority by date of receipt of entry, as long as the request is made with the entry, but not just prior to the tournament.
  • Confirmation of entry will be mailed within 2 working days.
